Psychological service
As part of providing a psychological service to you, Top Care Psychology needs to collect and record personal information from you that is relevant to your situation, such as your name, contact information, medical history, and other relevant information as part of providing psychological services to you. This collection of personal information will be a necessary part of the psychological assessment and treatment that is conducted.
Reason we gather and store personal information:
This information is obtained and stored for purposes of providing you with treatment. This information is kept secure and only used by your psychologist and administrative team when required for the purpose of providing you with psychological support and conducting necessary administration. The information you provide helps the psychologist to be informed and provide you with appropriate psychological services.
A detailed description of how your personal information is managed and how you can submit any questions or complaints about this service will be provided to you by your health professional upon request.
Disclosure of Personal Information
All your information gathered and stored for the purposes of providing you with a psychological service, will remain strictly confidential except when:
Your information is subpoenaed by a court or required by law.
As a psychologist, I have a duty of care to disclose information to the relevant authorities if it reasonable to consider, you or someone else is at serious risk to life, health, or safety.
Where you have provided permission to have your personal information shared to third parties, such as Gp’s, case workers, social workers, lawyers, or others for the purposes of providing you with collaborative support or providing you with psychological services.
Your personal information will not be used, sold, or disclosed for any other purpose.
Consequence of not providing personal information
If you choose not to provide you personal information, or not provide permission for this information to be collected for the purpose of providing you with psychological support, Top Care Psychology may not be able to provide psychological support to you.
Provision of telehealth services:
When required, psychological services will be provided by telephone or video. To have telehealth psychology sessions, you need to be situated in a private, quiet space. You need to have a phone, laptop or tablet with a camera and speakers to have video consultations, as well as a reliable internet connection.
There are risks associated with any online form of communication, however, we use secure software that meets the Australian requirement of online encryption and security.
Limitations of telehealth
Telehealth services have their limitations, such as not having a stable internet connection, or not having a quiet space to have your session. In addition, there are some treatments that are not appropriate or as effective when delivered via telehealth. Your psychologist will establish whether this method of treatment will be appropriate for you and your circumstances.
