Session assessments

​Initial session

Prior to your initial session, you will be sent the links to complete the intake form and the Depression, Anxiety and Stress Scale (DASS). You will also have the opportunity to attach your referral and MCHP (if it has not been emailed to us by your GP). You are expected to complete the forms and provide the MHCP (if applicable) before the initial session. 

Subsequent sessions

You are expected to complete the DASS before every session with our psychologist. After every session, you will receive the Outcome Rating Scale (ORS) and Session Rating Scale (SRS). We will appreciate if you can complete them immediately after sessions as this will help us keep track of your progress and improve the quality of care provided in our sessions. Please do not hesitate to discuss any concerns you may have with your psychologist during sessions.
